Attendees must register for The Revolution & Paradigm Shift in Treatment of Migraine to book room reservations at the discounted group rate at Disney’s Grand Floridian Resort & Spa. The discounted group rate is $319 per night, plus taxes (single/double occupancy) and is based on limited availability. The booking link is currently open and will be sent to the email address provided upon registration. The group rates are available to registered attendees on a first-come, first-served basis or until the room block is sold out, whichever comes first. The deadline to book room reservations at the group rate is June 12, 2023. Reservations made for days outside of the block or made after the sold-out date/booking deadline will be based on the hotel’s current rate. Reservations will be verified with the registration list. Reservations made by non-registrants will not receive the discounted group rate and may be cancelled.

Diamond Research & Educational Foundation will cover your hotel stay (room rate and tax) for the days/nights you are scheduled to present. Please complete your Faculty Room Reservation Form as soon as possible so that your reservations at Disney’s Grand Floridian Resort & Spa can be made to reflect your request. Fees for extra days/nights outside of your scheduled lectures and additional guest fees will not be covered and will be at your own expense. You will receive your hotel confirmation email directly from Walt Disney World Resort at info@cvent.com.
To book your airline reservation, please contact Barbara Witkowski at Rex Travel. Please indicate that you are a speaker for the Diamond Research & Educational Foundation. To obtain the best possible rate, we ask that you book your airline reservation at least 30 days in advance. All reservations will be reviewed and paid by DHCREF. Any changes made after the reservation has been approved/ticketed will not be covered by DHCREF unless pre-approved.
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Primary Care Network (PCN) and the Diamond Research & Educational Foundation. Primary Care Network is accredited by the ACCME to provide continuing medical education for physicians.
Primary Care Network designates this live activity for AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending.
To receive your CME certificate or your certificate of participation, you must complete the Primary Care Network online evaluation form. The link to access the online evaluation will be emailed to all registered attendees who checked in and attended the conference. The link will be emailed after adjournment on Saturday, July 15, 2023. Your certificate will populate immediately upon completion of the evaluation so please be ready to print and/or save your certificate at that time. Primary Care Network will also send you a copy of your certificate to the email address provided on the evaluation form. The deadline to complete the online evaluation and obtain your certificate is July 4, 2023. The online evaluation system will close after this date, and no certificates will be issued. Email reminders will be sent out weekly until the deadline. Please be sure you don’t miss the opportunity to receive credit for your participation.
The Diamond Research & Educational Foundation is pleased to invite abstract submissions for poster sessions at our live conference: The Revolution & Paradigm Shift in Treatment of Migraine. This conference will be held July 13-15, 2023 at Disney’s Grand Floridian Resort & Spa at Walt Disney World®. The deadline for abstract submissions is May 22, 2023 by 11:59 PM (CST). All abstracts will be reviewed by the Review Committee and may be accepted or rejected for poster presentation. Notification of acceptance and instructions will be sent to the submitter via email by May 29, 2023 or sooner. Abstract submission constitutes a commitment by the author(s) to present their work if abstract is accepted. Poster presentations will be displayed throughout the course – refer to the program agenda for the schedule.
