CLAIM CREDIT/CME CERTIFICATE:

To receive your CME certificate or your certificate of participation, you MUST complete the Primary Care Network online evaluation form. You may access the evaluation form at:
https://primarycarenetwork.org/headache_update_2021/. Your certificate will populate immediately upon completion of the evaluation, so be ready to print and/or save your certificate at that time. A copy of your certificate will also be emailed to the email address provided on the evaluation form. Evaluations must be completed by Friday, July 30, 2021. The online evaluation system will close after this date, and no certificates will be issued.

Abstract Submission:

The abstract submission deadline has passed. All abstracts received have been reviewed by the Review Committee. Notification of acceptance or rejection for poster presentation has been sent out to the original submitter via email. 

Accepted Abstracts for Poster Presentation:

All poster presenters are required to register & attend the full course. Poster Presenters are responsible for set-up and take down of posters at designated times and should be available for questions/inquiries for poster sessions throughout the course. Poster numbers are assigned on a first come first serve basis.
